You have important rights related to your mental health services. This notice explains how to:

  • Request your health care records from this provider

  • Contact the Texas Behavioral Health Executive Council (BHEC)

  • File a consumer complaint with the Texas Office of the Attorney General’s Consumer Protection Division​

 

Request your health care records

To request your health care records from this practice:

  • Submit a written request (letter, secure message, or portal message, if available) that is signed by you or your legally authorized representative.

  • Include your full name, date of birth, contact information, dates of service (if known), and a brief description of the records you are requesting.​

File a consumer complaint with the Texas Office of the Attorney General

You may file a consumer complaint with the Texas Office of the Attorney General’s Consumer Protection Division if you believe a business has engaged in deceptive or unfair practices.​
